"Freezing" it so a user can't edit is a good idea but going back to my original post the issue is moderators unaware of the actions of other moderators.I went down the route of 'freezing and unfreezing' the topic so moderators can work on the topic but then I wasn't sure to what degree should 'freezing' a topic prevent other users from accessing it.
Two moderators open same topic around the same time, one decides to move it to forum A and a second later another moderator moves it to forum B.
Haven't tested this but I would suspect this is issue. Two moderators open a topic and hit post reply around the same time. First one to submit locks it and the second one accidentally unlocks it with the post.
Two moderators open a report about the same time and both send the user a warning.
Other moderators should be frozen out too if you are going to implement something like that. Whichever moderator takes "ownership" of the entire topic should be the only one able to make changes for X time. Other moderators should just see a flag indicating it's being worked on.
